Isn't lip fat grafting unnatural?
Because a small amount of micro-purified fat is grafted, it's natural. Avoiding excessive volume is the NB principle.

Can I get it with filler?

Does the philtrum length also improve?
When lip volume is restored, there's an effect of the philtrum looking relatively shorter. It is not a direct philtrum-shortening surgery.
