DIAGNOSIS
Even the same ‘heavy eyes’ have different causes
It may not be a matter of skin sagging alone. We analyze sagging, fat thickness, eye-opening strength, and brow position together to set a direction that fits the cause. Misreading the cause makes the result look awkward.
Skin sagging
We look at how much the sagging eyelid skin covers the field of vision. Too much resection becomes unnatural.
Fat thickness
We check whether the fat above the eyelid is thick and looks heavy. It may not be a matter of skin alone.
Eye-opening strength
If eye-opening strength has weakened along with drooping, upper blepharoplasty alone may not make the eyes distinct.
Brow position
We distinguish cases where the brow has descended together. If it doesn't match the cause, the result looks awkward.

We look at preserving the existing line first
We preserve an existing double-eyelid line or natural eyes as much as possible. Rather than making a new one, we prioritize restoring the original impression.
INCISION APPROACH
Upper blepharoplasty is not one thing; the direction changes with the incision location
The direction of the result changes depending on where the sagging skin is refined. Rather than one method being better, it is important to choose the incision location that fits your eye condition and concern.
Type A
Double-eyelid-line upper blepharoplasty
Incision along the eyelid line
Advantages
- The shape of the double-eyelid line itself can be improved
- Sagging skin is refined at the double-eyelid line
- Orbital-fat removal / soft-tissue refinement can be done together if needed
- Suited when the line has faded or the shape needs improvement
Limitations
- Recovery and settling time may take somewhat longer depending on age and constitution
- To also improve outer-corner drooping, the incision may extend outward
- An excessive resection amount / line height can look artificial, so conservative design is needed
Type B
Sub-brow upper blepharoplasty
Sub-brow incision
Advantages
- Thick sub-brow skin can be resected to reduce drooping
- Little change to the existing double-eyelid line and natural eyes
- Sagging skin can be corrected naturally, mainly
- Can be especially advantageous for improving outer-corner drooping
- For older ages, the recovery burden tends to be relatively less than a line incision
Limitations
- Improving the shape of the double-eyelid line itself is difficult or limited
- If line correction is needed, a separate correction may be required
- As the scar is beneath the brow, the scope is decided considering brow shape and skin thickness

Q. Can my existing double-eyelid line be preserved?
Where possible, we preserve the existing line as much as possible in the design. We guide the possibility of preservation in consultation, depending on the degree of drooping and skin laxity.
Q. Will a scar remain after surgery?
Because we incise along the eyelid crease line, over time it becomes naturally hidden in the crease.
Q. Can eyelid drooping recur?
It can droop again due to aging, but the surgical effect usually lasts 10–20 years or more.
