We first distinguish whether a wide-looking cheekbone comes from bone structure, the soft tissue and subcutaneous fat around the cheekbone, or fibrotic fat. This is a different approach from bone-shaving surgery of the cheekbone itself.
Designing to preserve the volume to keep
Over-reducing cheekbone soft tissue can make the mid-face look hollow. Looking at the upper-outer facial width and the cheekbone-to-cheek transition, we design the volume to preserve together.
Checking skin elasticity and mid-face transition
We look together at the thickness of the subcutaneous fat layer, skin elasticity, and the transition from below the cheekbone into the cheek. If elasticity is insufficient, tightening can be considered as a complement.

It may be the flesh and soft tissue around the cheekbone — not the cheekbone itself — that makes the face look wide and heavy. We check together cases that do not reduce easily with dieting.
